When should a biopsy be performed?
When a lesion persists for more than 2–3 weeks, or in the presence of masses, ulcerations, or suspicious changes in color or texture.

Benign lesions (fibromas, epulis), premalignant lesions (dysplasias), inflammatory lesions, or oral carcinoma.
What does the histopathology report include?
Structured histological diagnosis, surgical margins if applicable, and differential diagnosis.
